What is Support Coordination
Support Coordination is a service that helps you acquire the skills necessary to understand and execute your NDIS plan. It is a funded service that may be included in your NDIS plan that provides ongoing assistance in connecting you to the appropriate services and supports.
Link Support Coordinators will help you optimise your NDIS funding and provide you with the information needed to make informed decisions about services.
Link SC offers two types of services that can ensure you receive the most suitable supports for your needs and allow you to have genuine choice and control; these are Support Coordination and Specialist Support Coordination.

Support Coordination Services
Support Coordination
This support will assist you to build the skills you need to understand, implement and use your plan. We will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
Specialist Support Coordination
This is a higher level of support coordination. It is for people whose situations are more complex and who require specialist support. We will assist you to manage challenges in your support environment and ensure consistent delivery of service.

What are the different levels of Support Coordination?
There are three levels of Support Coordination:
– Support Connection (Level 1): Basic assistance to connect with informal, community, and funded supports.
– Support Coordination (Level 2): More in-depth assistance to help individuals manage their support plans and connect with services.
– Specialist Support Coordination (Level 3): High-level support for individuals with more complex needs, requiring specialised skills to manage challenging situations and coordinate multiple services.
How do I switch to Link Support Coordination?
Not all Support Coordinators are the same, and it’s important to find one that aligns with your specific needs and goals. As an NDIS participant, you have the flexibility to switch Support Coordinators at any time during your NDIS plan. If you feel that your current Support Coordinator isn’t meeting your expectations, consider making a change.

We’ll reach out to your current NDIS Support Coordinator to arrange the transition and notify all your service providers that we are now your new Support Coordination contact.
Do I need to tell the NDIA, my Local Area Coordinator (LAC) or Early Childhood Partner (ECP) if I change Support Coordinators?
If you’ve had a Support Coordinator in your current plan, you already have funding for Support Coordination. There’s no need to contact the NDIA or your Local Area Coordinator or Early Childhood Partner, as there’s nothing they need to update or amend. All you need to do is choose the Support Coordination provider you’d like to work with.
How do I know if I am eligible for Support Coordination?
Eligibility for Support Coordination is determined during the NDIS planning process. If you have complex needs that require assistance in coordinating your supports, your NDIS plan may include funding for Support Coordination. The NDIS planner will assess your individual circumstances to determine the appropriate level of support.
How do I get Support Coordination in my NDIS plan?
If you don’t already have funding set aside for an NDIS Support Coordinator, you’ll need to explain to the NDIA why you need one. Some key reasons could be that you’re going through a major life change, you don’t have a support network that can help, this is your first NDIS plan or you are having difficulty setting up supports to implement you plan.
There are three levels of NDIS Support Coordination available, which you will discuss at the initial planning meeting. Your local area coordinator (LAC) or Early Childhood Partner (ECP) will help determine what you’re eligible for.
